Home Care Packages are a planned and case managed package or group of services, to support you to remain living independently at home.

My Aged Care

The Australian Government understands that most people prefer to live independently at home, but need some help with daily tasks.

Home support services and home care packages are federally funded and may be available to you. If you are just starting out, call us on 1300 363 654.

Who Is Eligible?

NovaCare provides support services to residents across the Hunter especially in the Newcastle, Port Stephens, Maitland and Lake Macquarie areas.

My Aged Care services and packages are for people aged 65 or over (or Aboriginal or Torres Straight Islander over 50) and have a current approved assessment by the Aged Care Assessment Team.

 

Steps to My Aged Care

Do you need a hand? We can guide you through the steps to access subsidised home support services and connect you with My Aged Care. Call us on 1300 363 654

An assessor will make an appointment to come to your home and discuss your needs. You may qualify for subsidised support through the Commonwealth Home Support Program or Home Care Packages.

When you are assessed, nominate NovaCare as your provider of choice.

If you receive a letter from the Commonwealth Government regarding a Home Care Package or Commonwealth Home Support, please call us and we will assist you.
